The Effect of Child-centered Play Therapy for a Nine-year-old Boy with ADHD Comorbid with Anxiety Disorder

Abstract
The purpose of this study is to investigate the effect of a child-centered play therapy on a child with Attention Deficit Hyperactivity Disorder (ADHD) comorbid with Anxiety Disorder. The subject is an eight-year-old boy and the play therapy consisted of 83 sessions for 2.1 years. At the first interview, he seemed to be anxious and confused and failed to make eye contact. He had problems continuously with his peers and teachers at his kindergarten and school, showing inattentive and impulsive behaviors. He spent his infancy in an unstable environment because her primary care giver could not provide a sensitive care. At the early stage of the therapy, he did not make proper eye contact and little interaction occurred between the child and the therapist. The major objective of the therapy was to address developmental difficulties of the child and build a new strong relationship based on the experience of understanding. Gradually, he could build a secure and receptive relationship with the therapist, based on which the child expressed his negative emotions. To assess the effectiveness of the therapy, both qualitative and quantitative analyses were conducted. The findings revealed that the play therapy contributed to decreasing the child¡¯s ADHD behaviors and anxiety through the stable relationship with the therapist. Self-esteem of the child was improved, decreasing the level of stress from daily life. Also, his mother¡¯s depressive symptoms were reduced and her relationship with her child was improved. His performance improved in the fine motor skill test, intelligence test, and K-WISC III. Furthermore, the results of K-CBCL, KPR-C, and Conners test showed that his ADHD symptoms and overall problem behaviors are within the normal level and that the score of other problem behavior scales also decreased.
